Your personal information (health status) is being collected by the individuals who you have provided email addresses for, as per section 26(c) under the Freedom of Information and Protection of Privacy Act.
No personal information is collected by this application.
The answers you provide to the Health Check questions are not retained in the application and are not forwarded to your supervisor, designated screener or the Public Service. You will receive an identical copy of the email sent to your supervisor or designated screener.
